What this page covers
This policy applies to stuckecounseling.com. It does not describe how protected health information is handled inside the practice. That is covered in the Notice of Privacy Practices.

Information collected
If you use the contact form, the information collected is whatever you type into it: your name, a way to reach you, and a short message. Nothing more is requested and nothing more should be sent.
Standard web server and hosting logs may record technical information such as IP address, browser type and pages requested.
- Do not send clinical details, diagnoses or crisis information through the form.
- The contact form is not a secure or encrypted channel.
- Email is not a secure channel either.
How it is used
Contact form submissions are used for one purpose: to respond to your inquiry and arrange a conversation. They are not sold, rented or used for marketing lists.
Submitting the form does not create a counselor-client relationship and does not make you a client.
Information is not shared with third parties except service providers required to operate the website and email, and where disclosure is required by law.
Inquiry messages are retained only as long as needed to respond and to keep a reasonable record of contact.
Your choices
You can call (812) 760-5528 instead of using the form. For anything clinical, that is the better option.
