Thoughts drive the response. So we go after the thought.

CBT is the only modality I practice. It is structured, it is testable, and it gives you something to use on a Tuesday when we are not in session.

The basic mechanism

Something happens. Before you feel anything, a thought runs, and it runs fast enough that most people never catch it. The feeling and the behavior follow the thought, not the event.

That is why two people can have the same week and one of them is fine. The event is shared. The interpretation is not.

The event does not decide the response. The interpretation does.

The four moves

Catch it. We slow the sequence down until you can hear the thought in your own voice, usually by working backwards from a reaction you did not like.

Test it. Is it true, is it the whole story, and would you accept that reasoning from someone you respect. Most of the thoughts running us do not survive being said out loud.

Reframe it. Not cheerful, accurate. We build the version that holds up under pressure and you practice using it when it counts.

Act on it. Nothing is real until you do something differently and see what happens. Then we look at the result together.

What a session actually looks like

There is a structure, and you will feel it by the second or third time. We start with what happened since we last met, specifically the moments that did not go the way you wanted. Not a general report on the week. One incident, with enough detail that we can take it apart.

Then we walk the chain backwards. Something happened. A thought fired, usually in under a second and usually in a voice so familiar you never registered it as a thought at all. Your body responded, because it always responds to the thought and not to the event. And then you did something: raised your voice, went quiet, left the room, poured one, sent the email.

The question I keep asking is which link has leverage. You cannot control what happened. By the time your chest is tight, the physical response is already underway. The behavior is the easiest thing to see and the hardest thing to change by willpower alone. The thought is the link that moves, and it moves once you can hear it.

You will be asked direct questions. If a session is drifting into a pleasant conversation that changes nothing, I will say so, because your hour is worth more than that.

Between sessions, and why it is not optional

One hour a week is roughly half a percent of your week. If nothing happens in the other ninety-nine, nothing happens.

So you leave with something to do. Usually it is small and specific: notice the thought when it fires and write down the actual words, run one test of a belief you have never tested, have a conversation you have been avoiding and report back on what happened rather than on what you feared would happen.

It is not homework in the school sense and there is no grade. It is data. When you come back and say it did not go as expected, that is the most useful hour we have. And when you come back having done none of it, that is useful too, because whatever stopped you is usually the same thing stopping everything else.

This is also where the honest answer about length lives. Two rules of thumb: you get out what you put in, and these issues did not get like this overnight.

What CBT is not, and who it does not suit

It is not positive thinking. Swapping an inaccurate harsh thought for an inaccurate cheerful one just gives you a second thing to disbelieve. We are after the version that is true and survives being tested.

It is not a lecture, and it is not a worksheet factory. The structure exists to keep the hour honest, not to turn you into a filing system.

It is not a promise. Nobody can guarantee an outcome, and anyone who does is selling something.

And it does not suit everybody. If what you want is an hour of unstructured company each week, this will feel demanding and you would be better served elsewhere. If you want someone to agree with you about the other person, we will run into trouble by the third session. Where the 40 percent rule fits

When you tell yourself you are done, you are at about 40 percent. That is the cleanest demonstration of the whole model: a thought reporting a limit that is not the limit.

Once you have caught your mind being wrong about that, you start treating other certainties as claims that can be checked rather than as facts.

It is also how resilience gets built. Every time you go past the thought that said stop, the next storm costs you less.
