Notice of privacy practices

What you say in session stays in session, with a small number of specific exceptions required by law. Those exceptions are listed below and they are not negotiable, so it is better that you know them going in.

A copy of this notice can be requested at any time and will be sent to you.

Protected health information may be used or disclosed for treatment, payment and health care operations, and where otherwise required by law.

Confidentiality has limits. The main ones:

• Suspected abuse or neglect of a child or an endangered adult, which must be reported.
• A serious and imminent threat of harm to you or to an identifiable other person.
• A valid court order or other legally compelled disclosure.
• Where reporting is otherwise required by Indiana or federal law.

Most other disclosures require your written authorization, including any release to a family member, employer, attorney or another provider, and any use for marketing purposes.

Psychotherapy notes, where they exist and are kept separately, receive additional protection and generally require specific authorization to release.

You may revoke an authorization in writing at any time, except where action has already been taken in reliance on it.

Under HIPAA you have specific rights regarding your record.

• To inspect and receive a copy of your record, subject to limited exceptions.
• To request an amendment to information you believe is inaccurate.
• To request restrictions on certain uses and disclosures.
• To request confidential communication by a particular method or at a particular address.
• To receive an accounting of certain disclosures.
• To be notified in the event of a breach of unsecured protected health information.
• To receive a paper copy of this notice.

Email, text messaging and the contact form on this website are not secure channels. Do not use them for clinical information.

Telehealth sessions are conducted using a platform intended for clinical use.
